Hyperglycemia, a risk factor for cardiovascular disease, is always your diabetic patient’s first priority. A combination of diet, exercise, drugs, and stress reduction can help your patient keep her blood glucose levels as close to normal as possible. Encourage her to monitor her blood glucose levels at home so that she can adjust different aspects of her treatment as needed. Keep in mind, however, that the physician may modify your patient’s treatment after a severe cardiac problem to achieve stricter blood glucose control. That’s because avoidance of severe hypoglycemia is crucial to preventing arrhythmias, which can lead to more serious cardiac problems, such as an MI.

Hypertension accelerates the vascular changes of already compromised coronary arteries. Monitor your patient’s blood pressure frequently. If her blood pressure exceeds 140/85 mm Hg on two separate occasions, she’s considered hypertensive. The physician may prescribe lifestyle modifications, such as a low-sodium diet, alcohol restriction, and regular exercise. If 3 months of nonpharmacologic therapy don’t reduce your patient’s diastolic blood pressure below 90 mm Hg, the physician may prescribe an antihypertensive drug, such as an ACE inhibitor. If your patient’s diastolic pressure exceeds 110 mm Hg or if she has microalbuminuria, the physician may prescribe an antihypertensive drug immediately in conjunction with lifestyle modifications.

Many patients with diabetes are at risk for CAD because they have hypercholesterolemia. Even those with normal or nearnormal total cholesterol levels may be at risk because their heartprotective, high-density lipoprotein (HDL) levels are abnormally low, and their low-density lipoprotein (LDL) levels are abnormally high. Typically, a low-cholesterol, low-fat diet is recommended for patients whose total cholesterol level exceeds 200 mg/dl and whose LDL level exceeds 130 mg/dl. Encourage your patient to reduce her total fat intake to less than 30% of her total calories and her saturated fat intake to less than 10% of her total calories. If 6 months of diet therapy and exercise don’t reduce your patient’s LDL level to 160 mg/dl or less, the physician may prescribe a lipid-lowering agent, such as lovastatin.

To help your patient reduce her weight, the physician may prescribe an individualized diet and exercise plan. The goals of the weight-loss program are to help improve your patient’s blood glucose levels and better control her blood pressure. She can achieve these goals with even a modest weight loss of 10 to 15 pounds. Help your patient understand that a large weight loss followed by a weight gain is stressful to her body. Instead, encourage her to maintain her weight and blood glucose levels by controlling her food portions, eating balanced nutritious meals, and eating her meals at the same time each day.

If your diabetic patient smokes, she should stop. To help ease her withdrawal symptoms, her physician may prescribe nicotine in a dermal delivery system or a chewing gum. The physician may also encourage her to attend a support group.

Drug Therapy

Your patient’s physician may prescribe one or more drugs to manage CAD and help prevent its complications.

To treat coronary artery insufficiency, the physician may prescribe a beta-blocker, such as propranolol. But if your patient is using insulin, she may not be able to take beta-blockers because these drugs can impair insulin secretion and alter or mask the signs and symptoms of hypoglycemia. Without the typical warning signs and symptoms of hypoglycemia-such as dizziness, diaphoresis, or nausea-your patient may not realize that her blood glucose level has fallen dangerously low. However, some patients who use insulin can tolerate low doses of cardioselective beta-blockers, such as atenolol, metoprolol, or acebutolol.

If your patient has had an MI, the physician may prescribe aspirin to reduce the risk of further complications of CAD.

If she has angina, the physician may prescribe a nitrate, such as a nitroglycerin patch. Although the nitrate won’t interfere with blood glucose control, it can produce severe hypotension. So monitor your patient’s blood pressure and teach her about the signs and symptoms of hypotension, such as light-headedness when changing from a lying position to a standing one.

If your diabetic patient has heart failure and hypertension, drug therapy may pose problems. If she also has autonomic neuropathy and orthostatic hypotension, calcium channel blockers may be in appropriate. Although ACE inhibitors have been used successfully in patients with diabetes, they can increase serum potassium levels, requiring close monitoring for signs of hyperkalemia. Thiazide diuretics can raise blood glucose levels and reduce potassium levels, requiring close monitoring for signs of hypoglycemia and hypokalemia. They may also cause impotence or orthostatic hypotension, especially in a patient with autonomic neuropathy.

Invasive Treatments

Patients with CAD may also benefit from invasive treatments. In those with coronary ischemia or MI, percutaneous transluminal coronary angioplasty can restore blood flow in blocked coronary arteries. However, if a patient with nephropathy receives radiographic dye during this nonsurgical invasive procedure, she may develop further kidney problems. Coronary artery bypass surgery may also be performed in diabetic patients. However, high glucose levels during surgery can increase the risk of postoperative complications and death.

Don’t Buy into These Exercise Myths

The past decade or so has seen an explosion of research about nutrition, exercise and the human body. Some of this research has confirmed what we already knew—things like, a healthy diet and regular exercise make you live long and prosper. Some research, however, has proven that some “common wisdom” about exercise is just plain wrong. Here are ten myths that you don’t want to buy into.

1. Lifting weights makes you bulk up. Actually, this is true—if you are male. Women don’t have the biochemistry necessary to develop big, bulky muscles. They will get stronger, but not bulkier.

2. It’s inevitable to gain weight as you age. False. People gain weight as they age because they become more sedentary and their metabolic rate drops. Weight gain is not inevitable.

3. Fat people are not fit, so you have to lose weight before you can get in shape. False again. Fitness has to do with flexibility, muscle strength and endurance and cardiovascular endurance, not with how fat you are. Fat people may not look fit, but some are in pretty good shape. Getting in shape will also help you lose weight faster because it makes your body burn calories faster.

4. No pain, no gain. Absolutely not. You should not hurt before, during or after exercise. Some mild, transient muscle soreness may occur the first few times you work out, but it should not persist.

5. If you don’t exercise regularly, there’s no point in exercising at all. Nope, not true. Of course, it’s ideal to exercise regularly, but anything at all is better than nothing at all.

6. Old people can’t exercise. Exercise has definite benefits for older people. Lush, green weed free lawns - they are the dream of every homeowner in America, but the price we pay for those luscious grass carpets may be more than the dollars spent on fertilizers, herbicides, pesticides, and fungicides not to mention the hours spent on mowing and trimming. The price may be your health and the health of our children and pets.

A study by Dr. John Peters of the University of Southern California (July 1987 issue of the Journal of the National Cancer Institute) showed that children living in houses where garden pesticide sprays were used on a regular basis faced a risk of contracting leukemia that is 61/2 times greater than normal. Another study published in the October 1991 issue of the same publication showed that dogs whose owners used a herbicide called 2-4-D were twice as likely to develop lymphatic cancer.

At last there is an alternative to chemical based lawn treatments. Leo Malantis, President of Millennium Lawns and a Consultant to the Golf Course Industry states: Fertilizers or Plant Foods alone cannot provide the biological balance to the soil and likewise; no biological solution by itself can maintain a nutritional balance in the soil. The soil is the stomach for the plant and it needs the the soil based organisms to break down the nutrients,to make them available to the plant in a digested form. Since many microbes are killed by herbicides and pesticides, it stand to reason that replacing them should be the first consideration of a fertilization program Malantis states, “The soil can only hold so much food, after that, the excess nutrients are wasted or even worse become toxic to the plants itself. Microbes are necessary in the soil for the bacterial decomposition of roots and stems that create organic matter. The whole chain of microorganisms must work in the soil to break this organic material into humus, which then becomes nitrogen, phosphorus and potash, which the plant takes up as food.

That’s the life cycle in the soil and too many chemicals can wipe it out.” Once your soil becomes lifeless the grass is dependent upon nitrogen, phosphorus, and potash from an outside source - Generally synthetic fertilizers purchased by the homeowner and/or applied by the lawn service companies.

SOIL TOXICITY
All fertilizers and herbicides are water soluble salts. Grass plants are like sponges and will absorb all nutrients available until they become saturated. Once saturated, like a wet sponge, they cannot absorb any more food and the excess nutrients, as much as 60%, will become nutrient run off that pollutes our underground water supply, or will remain tied up in the soil as a salt. This salt build-up prevents the roots from penetrating the soil begin to feed at the surface of the lawn causing a compact soil and mat and thatch.

SOIL COMPACTION
Ideally, a “friable” soil is composed of 45% minerals, 5% organic matter, 25% air and 25% water. Roots in the soil decompose to form the organic matter that is the glue that holds the air and water in the soil. Without organic matter, the soil readily compacts CORE AERATION MYTH “Core aeration is not a solution,” states Malantis. Soil compaction takes place deep in the soil as clay, silt, and sand, lacking organic matter, begins to stratify. When you core aerate you will bring up soil cores that contain weed seeds. Once exposed to surface heat and moisture the weed seeds germinate and grow requiring a chemical herbicide to control them.

MAT AND THATCH
The compact soil now has forced the roots to feed at the surface of the lawn. This mass of roots at the surface is called mat & thatch. Mat & thatch has a number of negative impacts on lawns. First: Mat and thatch limit the rhizome growth and result in a thin lawn. Secondly, weed seeds wilL germinate more readily in a thin sparse lawn, Thirdly: Mat & thatch becomes a breeding ground for insects and disease, DETHATCHING MYTH. Dethatching is another myth that Malantis attacks. “Thatch, as we said before is organic matter. Thatch should be decomposed with enzymes and returned to the soil, not removed to a waste dump.”

ORGANIC-BASED SOLUTIONS

Adjust your lawn mower to its highest or second highest level and never cut more that 1/3 off of the blade at a time, avoiding the possibility of the lawn going into shock. It’s better to mow more often and leave the grass clippings on the lawn to decompose and return to the soil as organic matter. Contrary to popular belief, grass clippings decompose rapidly, and do not contribute to mat and thatch.

WATERING
Regular water saturation of the soil is a necessity for deep root growth. Frequent, light watering will result in shallow roots. It is recommended to water about one inch on the surface of the lawn. This can be measured by setting a rain gauge or coffee can on the lawn and timing how long it takes to fill to a one inch level. Lawns should be watered once a week through out the growing season or more frequently if weather is extremely hot for long periods. During dry spells if the lawn cannot be watered regularly, it is better to let lawn go dormant rather than water infrequently.

Thinking Through Recycling Myths

Recycling is one of the things people think of first when they think about being environmentally friendly. In some ways that’s a pity, as reduce and reuse really should come first, but recycling is nonetheless important, and one of the easiest for the government to encourage both individuals and businesses to do.

Recycling has improved greatly these past several years, but it’s still not great. Far more could be recycled, especially when it comes to paper. It is still easier in many areas to throw paper in the regular trash, whether you’re at home, work or out and about.

Myths about recycling abound, as many people don’t really think carefully about it.

One is that recycling is hard. This one really depends on where you live. Where I live it is ridiculously easy for the materials that are accepted. One bin, no sorting required. Other places require sorting, but do provide the bins. It’s not that common anymore for recycling to be difficult.

Some also claim that recycling takes up just as much energy as using new materials.

How close this comes to being true depends on the product. Glass and metal recycle fairly easily. Plastic is more difficult. In fact, recycled plastic generally costs more than virgin plastic.

Plastics in general can be truly confusing. Which can easily be recycled depends upon the area you live in. Where I live, only #1 and #2 plastics are accepted for recycling. Other places accept a much broader range. I truly wish we had such a program here, but we do have to work with what we have in a given area.

Recycling in general costs money, often more than merely dumping the trash. Really this should not come as a surprise; there’s more effort involved. But it appears that it may not be as cost ineffective as some have claimed.

Then there are the claims that there is plenty of landfill space. This is combined with the idea that recycling is expensive, wasteful and ineffective to make the point that recycling is nothing more than a green myth itself.

So how available is landfill space?

I’m not going to mess with the claims that all we need is a single landfill x square miles in area and however many feet deep. That’s just not practical. Rather, I will look at how we are doing currently in terms of landfill space and over the next few years. To do this, one must also consider location. It is far easier to find landfill space in some areas than it is in others. The East Coast in particular has this difficulty.

Privatization has helped the landfill space issue tremendously. It’s not in the crunch it once was. And many places have years and years left in their landfills. But they are not infinite.

And then there’s how you make a landfill. The land used in landfills could be left natural or used for other purposes. I cannot think of any benefits to not trying to fill them more slowly. As landfill space becomes more scarce, the costs of using them go up. It won’t always be relatively cheap to throw out garbage.

Some also assume that biodegradable means that it will break down within a reasonable time. This doesn’t work in your typical landfill, where everything quickly gets covered up and decomposes in an anaerobic process. Most biodegradable items need air to really break down, and produce methane as they do.

Best thing to do with truly biodegradable items is to use them in your own composting. You can have the control over them so that they will break down into something reusable. You will want to be certain that you are using things that will be friendly to your garden, of course.

Acne treatments and so called acne solutions can sometimes be disappointing. The fact is the skin is the largest organ of the body and it and it needs proper nutrition to function properly. One thing is for certain we are what we eat and our health position is generally a reflection of our diet. Washing affected areas twice a day with sulfur based soaps specially designed for acne is highly beneficial. Sulfur is also an effective treatment of inflammatory pimples and acne lesions. Washing at the affected area first thing in the morning and before bedtime is with a product containing this component can prove over time to be effective.
